Information on this form is held in confidence and is not shared without your permission. With that said, information shared in this form is subject to be seen by: the Pastor, the Director of Religious Education, the Adult Formation Coordinator, the Marriage Coordinator, and the Sacramental Records Keeper. Your information may be shared with the Marriage Coordinator if you are seeking marriage, convalidation, annulment, etc.

I understand that if I DO NOT give St. Odilia permission to use photos or videos of my child(ren), I need to send a labeled photo of each child to the Faith Formation Office. Fees & Payment

We suggest a $40 donation to cover the cost of books and other materials. This donation is not mandatory, but encouraged.
