PARENTAL AUTHORIZATION, RELEASE & INDEMNITY AGREEMENT I, parent or guardian of the above child (ren) grant permission for my child(ren) to participate in this parish/school event. This event will take place under the guidance and direction of parish/school employees and/or volunteers from St. Odilia Catholic Church. I understand and agree that as parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above-named minor (“student/participant”). Further, I hereby warrant that to the best of my knowledge, my child is in good health, and I assume all responsibility for the health of my child. Description

If you are a youth entering Grades 6-12 in 2024 (and under the age of 18) then please fill out this form to volunteer for VBS. Please fill out 1 form per youth volunteer. We can accept 90 volunteers this year and will accept volunteers on a first come, first served basis. While we will work hard to allow volunteers to work in their preferred positions, given the large volume of campers and volunteers, preferred volunteer positions cannot be guaranteed.

There will be no charge for youth volunteers this year.

There will be opportunities to provide some donations for our After Hours days. Some items may include puffy paint, chips, juice boxes, etc.

After Hours is included in this sign up. Leaders are encouraged to stay after VBS on Monday and Thursday for their own time of both leadership training and fun activities/food.

Each volunteer will need 2 character references. This form will be emailed out to you shortly after you register. You may email the link in the email to the references, and they can fill it out online! No need to turn anything in.

Code of conduct forms will also be emailed out. They can be signed and brought to the youth training.
